- Qualified Medicare Beneficiary (QMB): Medicare A & B premiums and co-payments = 100% FPL (Federal Poverty Level)
- Specified Low-Income Medicare Beneficiary (SLMB) : Medicare B premium = 120% FPL
- Qualified Individual – 1 (QI-1) : Medicare B premium = 135% FPL
- Qualified Disabled Working Individual (QWDI): Medicare A premium = 200% FPL
