Reporting Suspected Public Assistance Fraud

Public assistance fraud is a crime! Fraud occurs when a person knowingly and purposely provides false information about their circumstance to obtain benefits. This is unfair to the taxpayers and the people who are really in need of assistance.

Cuyahoga Job and Family Service Office of Investigations’ goal is to reduce fraud in all public assistance programs. The Investigations Office accepts referrals and conducts investigations on all complaints of suspected fraud with the intention of detections, prevention and prosecution. If you know or suspect someone is misusing Public Assistance benefits, you can report it to us by using one of the following methods:

Requesting Replacement SNAP EBT Card Benefits Due to Electronic Theft

To report electronic theft of Supplemental Nutrition Assistance Program (SNAP) Electronic Benefits Transfer (EBT) card benefits, you can complete the JFS 07011 Form, also called the  “Statement Requesting Replacement of Electronically Stolen SNAP Benefits”. To complete the form, follow these steps:

  1. Visit the Ohio Department of Job and Family Services Forms Central Page.
  2. Please type the form number “07011” into the search bar to find the fillable pdf of the form.
